Important Health Coverage Tax Documents – Affordable Care Act 1095-c Notice

IRS Form 1095-C contains information about your employer-provided health coverage.
If you are an active Patterson Employee, you have access to your Form 1095-C in Benefitsolver. Both active and terminated Patterson employees may also request their form through email at hrcompliance@pattersoncompanies.com, by phone at 833-520-9740, or by written request to Patterson Companies, Inc. at 1031 Mendota Heights Road, Saint Paul, MN 55120. Your requested form will be furnished within 30 days of receiving the request. Form 1095-C is not required to file with personal taxes.
